Who We Are
Since 1972, the Council on Aging has been providing services to keep seniors, 60 and over who reside in Lake County, in their homes longer where they feel most comfortable and safe, along with offering support to their families and caregivers. This provides aging adults and their support system with services and opportunities for them to achieve an optimal, self-defined, quality of life.
How to Get Started
A resource specialist is available to answer your questions. Call us at 440-205-8111 and ask for a resource specialist. You also may email screener@lccoa.org. If our resource specialists cannot fully address your concerns, a case manager will call you and arrange for an in-home visit. At the visit, the case manager will talk with you, and anyone you want present, about your current situation and how you are managing, including your health, safety, nutrition, finances, housing, daily activities and any other issues. You will then decide how you want to handle your concerns. Together, you and the case manager will develop a care plan, including information, benefits and programs, and services that you want from the LCCoA or other agencies. The case manager will help make necessary arrangements and will follow up with you as needed.
